I had said the VMO b/c the pain is superior to the knee - it is more in the middle of my quad, slighly medial but not enough to be the adductors - plus it doesn't hurt with adduction, only back and forward movements of my leg. It may be something else, but it does not go down to my knee - stops about two inches superior and goes up about 4". It started as a mild twinge during my long run last week, and has gradually gotten a little worse. Shoes are fine. Hips are a little tight with the leg length discrepency (had my sports med doc check - for now leave alone, stretch, and if I start having problems try very small inserts into my right shoe to even it out gradually. I'm about 3/4 cm short on the right).
