Never heard of this either until it happened to me...had to have it surgically removed about 5 years ago. Doc said no cause (this was before serious biking) just "bad luck". Didn't hurt, just weird and bothersome and potentially can grow larger with time. Was an out-patient surgery, not much of a big deal all in all.
