Twice. Once the first time clipping in. Landed on my poor mother's foot and broke her toe with my elbow. The second time was a year later and I had no excuse. I wasn't paying attention and looked back to check on a friend while unclipping and went down right in front of an elderly woman going for a walk, who made a big fuss about making sure I was okay.
