My road bike is custom. The fit was included and fit like a glove from the first.
Before that my first road bike (my namesake) was fit over time by the same LBS. There was lots of trial and error, replacement and adjustment, "*&^% my shoulders hurt, can we do something?".
I learned a lot by that process but oh the painful rides to reach a customized fit.
Yes, fitting the bike is worth it. Go for it.
