Yeah, I was going to say freehub. If the problem comes back, though, try wiggling the wheel side to side with your hand with the bike sitting still, wheel on the ground. Is there a lot of lateral play? This is different than the wheel being out of true. It might be that the outsides of the hub itself has to be tightened (not the freehub). I'm told this is fairly common with Mavic wheels.
Some Easton hubs will also do this, but it's more difficult to do it, and it's not a long-lasting fix (eventually the whole hub needs to go).
Mavics do need to have the freehub pawls regreased pretty regularly. How old was that freehub? Just curious to see how much life you got out of it.

It was Mavic (Aksium wheels, so probably a lower grade freehub than what they put on the Ksyriums), and it had over 10K miles. I'd estimate it had been exhibiting the more minor symptoms for at least 1K before it finally died. The hubs have been serviced twice in that time and the freehub at least twice - they may have skipped it this latest time though.
I never checked the wheel hub for play, but I can't imagine I wouldn't have noticed that if it were there - wouldn't it cause squirrely handling, vibration on braking, etc.? I do check the headset and BB now and then, but I've never had one come loose without being able to feel it while riding. Anyway I'll add that to my list of things to check periodically.
