I don't know about 167.5, but the first 165 mm cranks I ever had (in 1987) were Campy... Record, I believe.
I had no trouble getting a 165 Shimano 105 crankset to swap out for the 170 that came on my complete bike. Lots of places seem to have them available in Dura-Ace and Ultegra too.
I think the dearth of hi-viz clothing is not limited to women's sizes. Most people just don't like to wear the stuff.
Can't speak to the rest of the stuff, but there's no harm in contacting the manufacturers/retailers and making sure they know what you're looking for. I know that motorcycling boots are very hard to get in the smaller sizes (even for my large-ish feet), and what's sold as "women's" boots tends to be lacking in safety features, so I feel your pain as far as shoes. What really gets me going is that they make them, and they sell them in Europe, they just don't import them to the USA.
