I've seen some really beautiful tats over mastectomy scars, flowers and vines and feathers, amazing creative lovely stuff. There's a woman here in Seattle who specializes in doing mastectomy scars. (she does other scars, too, and regular tats; but she's well known for her mastectomy scar work in particular) 2nd one, top row, morning glory, is one of her mastectomy pieces:
