Deb's response is 100% correct, however, if you are still having trouble trying to hold the sleeve the screw sits in while you unscrew the end screw, you can take off the metal plates and pedal platform, exposing the internal axile and lube port -- now it is easy to use needlenose pliers to hold end and unscrew cap, grease, and replace.
