    Hmmm, I guess it depends on what information you are trying to collect.

    Are you trying to understand why people buy and/or use helmets?
    Are you trying to collect usage based on age?
    Do you want information on designing a helmet that would increase usage?
    Are you interested in what age groups helmet use might drop off and then increase, again?

    As we say, or ask, at work, what is/are the question(s) you are looking for answers to?

    All the best, sound like a fun project.

    Oh what a timely subject! I just went helmet shopping today after a crash and it was a right mess. I have a very small head and it either was a $100.00 helmet (and as much as I ruin them, not a bright idea) or a helmet that did not fit! I finally got one that worked, but it was a chore.

    Threadless is a t-shirt company--anyone can submit designs, which are then voted on by the public, and top vote-earners are printed on shirts, hoodies, bags, etc. I believe the survey is referring to applying that sort of community model to bike helmet designs.

    Also note that it says "If you'd like" to get updates, to share your contact info--doesn't seem like that's mandatory to participate in the survey.
