    I just got one for my bike. (well, I shouldn't say "just", I got it about a year ago)

    Surly made 'em for the folks who had hangers that put too much of a kink in the brake cable.

    It does make the brake a lot smoother, and it seems to be kinder to the cable housing.

    tough to put a hanger like that on most small frames, as you run into interference with the brake straddle carrier as the rear wheel rim is much closer to top tube/seat tube junction....
