I wear a 4A and have a really hard time finding shoes, in general, due to low volume. I really like my Sidi 5.5's. The ratchet is adjustable on both sides and it keeps me from being all the way up the wrungs (?). My heels do slide when I walk in the 5.5's but they do not slide while I am riding. I bought them because I heard that Sidi ran more narrow than others.

I would think that insoles would really help you. I used Sidi insoles in my old Shimano shoes before I got new shoes.
