I LOVE LOVE LOVE Sidi. Wear the Genius 5 road and the Mountain Dominators that are the same shoe. Here are my thoughts:
1. They are narrow, if you don't have narrow feet you may not like them.
2. They fit very secure and hug your feet, but they are easily adjustable if you feet swell. I can even adjust them on the bike.
3. Their insoles are terrible and cheap, not much support. It doesn't bother me on my mountain bike but on my road bike I needed more arch support and went with Aline ortho insoles.
One complaint I have heard is they were sold out a few years back and people claim they aren't as great as their price. They used to be made in Italy and were supposedly better but I don't know. My road shoes have over 4,000 miles and are still great, I love them. All the complaints I have heard are from mountain bikers, we are rougher on everything so I expect my mountain shoes to wear out sooner than road shoes.
Yep, they are narrow...but the heels aren't particularly narrow. I had a very pretty pair of metallic blue Sidi Dominators. I ended up selling them after about a year, as I was starting to develop nerve problems in my feet from them being compressed in a too-narrow pair of shoes. I have a very wide foot. I now wear heat-moldable Louis Garneau men's shoes (I have both the road and mtn. version of the same shoe, I love them so much), which really fit like a dream AND have a narrow heel...I have duck feet. :p
Sidi wasn't sold, the American distributor was. Sidi's are made in Romania but the company maintains their quality control.
I have an older pair of mtn bike shoes and a pair of road shoes I bought last year and just got the white ones this year and they all fit the same. Can't say that for other companies.
I have the shoes you are talking about, I think, and I've been very happy with them. I do use E-Soles in them. They are white and they're not as hard to take care of and keep clean as I thought they'd be.
I love the adjustable strap over the instep, gives me more room over my high instep.
I too have the Sidi Genius 5 Pro carbon with e-soles -- I have the black ones. I like them a lot. Make sure to get the sizing right. On normal street shoes, I wear a 39, but on the Sidis I wear a 41.
I wear the SIDI MTB dominators and I love them and I do not have narrow feet. They fit me so nicely!

My sneakers are New Balance size 9D, but my Sidis are perfectly comfortable.
What is an e sole? I usually wear a 6.5 in regular shoes, but wear a 39 in cycling shoes.
It's a brand of insoles that you heat in the oven before inserting them into your shoes. I have and like them.
I have the same shoe you're considering. I like them, but I would suggest trying them on before purchasing. They only fit a specific type of foot--as others have mentioned. I otherwise like them quite a bit.

The neat thing is that you can chose the level of arch support and whether to have metatarsal support or not. A bit pricey, but they will last many years in my shoes.
I have two pairs of Sidi's, both men's because i can't stand flowery shoes. I gave the Sidi rep at a big bike store a hard time about that a few years ago because they did not offer black shoes for women. Not that he could do anything about it, but it felt good to at least express my disappointment.
I have the mountain bike shoes (not the Dominators because those are out of my budget, but someday...) and the winter boots, which I picked up used here on the TE forums. Both are great shoes.
